• Check-In 15 minutes prior to appointment time.
• Bring your current Insurance Card(s).
• Bring your Driver's Licence or Picture ID.
• Bring any referrals and/or medical records that are required for your visit
• Bring appropriate method of payment (cash, check, credit card). Co-pays are due at time of service.
• Bring your current medication list.
• Bring your completed New Patient Forms.
